To use an image in an email template it must be web image, you add it to the email template using the web address. To create a web accessible image see the article "Creating a Web-Image from a File"
From the Email Template record edit the HTML value section
To add an image click the image icon and select Web Image.
Enter the URL for the image
Save the email template.
